NEW ONLINE USERS

Please visit the practice in person with 2 forms of documentation as evidence of identity which will need to include a photo and your address.  

You must be aged 16 years or over to gain access to your online medical record.

ACCEPTABLE PERSONAL IDENTIFICATION DOCUMENTS:

  • Current UK Passport or EU/other nationalities passport
  • Current UK Driving licence photo card
  • National ID card and/or other valid documentation relating to immigration status and permission to work

ACCEPTABLE DOCUMENTS PROVIDING PROOF OF RESIDENCE:

  • Recent utility bill
  • Recent Bank Statement containing current address
  • Current benefit book/card or original notification letter from the DWP
  • Current local council rent card or tenancy agreement
  • Buildings insurance
  • Vehicle insurance

You will be asked to complete a form for Access to GP Online Services, you will then be provided with details via a printout or email to enable you to sign up to the online services.

ONLINE ACCESS (TEENAGERS) POLICY

Parents or Guardians of children under the age of 11 can register the child for online services to order prescriptions.  When the child reaches the age of 11 their account will be disabled.  The child will need to register to access the service in person when they reach the age of 16.  The log in details and password will only be given to the child.
