If you require any vaccinations relating to foreign travel you will need to make an appointment with the practice nurse.  Prior to this you will need to complete a Travel Vaccination Questionnaire (see below) print and sign it before bringing it into the surgery for the nurse to check.   The Practice Nurse will review your questionnaire, this will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

Please allow at least 3 days after submitting your form to see if any vaccinations are advised and to make an appointment with our nurse if necessary.

Please allow at least eight weeks before you travel.  Some vaccines require more than one dose and will not be effective until 2 weeks after the final dose.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
